London, UK: Iris hooks and pupil dilation rings are effective when used in patients with small pupils during cataract surgery, according to a new study published in the Journal of Cataract and Refractive Surgery. However, when using a pupillary dilator, the procedure time is reduced.
        Paul Nderitu and Paul Ursell of Epsom and St Helier University NHS Trust, London, UK, and colleagues compared an iris hook and a pupil dilation ring (Malyugin’s ring) in eyes with small pupils. Data from 425 cases of small pupils were evaluated with respect to duration of surgery, intraoperative and postoperative complications, and visual outcomes. A retrospective case study involving trainees and consulting surgeons.
        Malyugin pupil dilation rings (microsurgical technique) were used in 314 cases, and five flexible iris hooks (Alcon/Grieshaber) and ophthalmic adhesive surgical devices were used in 95 cases. The remaining 16 cases were treated with medication and did not require pupillary dilators.
       “For small pupil cases, the use of the Malyugin ring was faster than the iris hook, especially when performed by trainees,” the study authors write.
        “Iris hooks and pupil dilation rings are safe and effective in minimizing intraoperative complications for small pupils. However, pupil dilation rings are used faster than iris hooks. dilation rings,” the authors concluded.
